Attention Medicaid Members
A new change for the 2020 Tax Year:
Form 1095-B is a tax form that reports the type of health care coverage or health insurance each individual in your household had for the previous year. For the 2020 tax year, the IRS determined that an individual does not need this information to complete and file his/her tax return.
For the tax year 2020, the North Dakota Department of Human Services will not be mailing the 1095-B forms to clients. Copies of the form are available Jan. 31, 2021 upon request through one of the following options:
- Call the Medicaid Eligibility Call Center at 1-844-854-4825
- Email your request to medicaidcc@nd.gov
- Sending a written request to the following address: North Dakota Dept. of Human Services, Medicaid Eligibility, 600 E. Blvd. Ave., Bismarck, N.D. 58505
Report Medicaid Fraud and Other Fraud
- To report suspected Medicaid Fraud, please call, 1-800-755-2604 and select Option 6 to speak with an attendant, or email: medicaidfraud@nd.gov
- To report other program fraud, call the Fraud Hotline at 1-800-472-2622 or e-mail amarshall@nd.gov
