Fargo, ND — The U.S. Department of Veterans Affairs Fargo Health Care System is proud to announce the opening of a Bismarck Home-Based Primary Care Program (HBPC) to qualified Veterans who live within a 50-mile radius of the Bismarck HBPC office.  Patti Burwick, RN, MSN, serves as the Program Coordinator/Nurse Manager for the Fargo and Bismarck HBPC Program.

HBPC provides comprehensive, interdisciplinary, primary care to Veterans who live in their own homes or in assisted living facilities by having the team of HBPC professionals come into the Veteran’s home so the Veteran does not need to come in to the clinic for primary care appointments.

The primary focus of HBPC is to provide longitudinal care for those Veterans who have complex, chronic, and disabling diseases.   HBPC is also an excellent solution for Veterans who have mobility/transportation issues.  The Bismarck HBPC Interdisciplinary team is comprised of a Nurse Practitioner, Registered Nurses, Occupational Therapist, Registered Dietician, Licensed Social Worker and a Program Support Assistant.

The Bismarck HBPC hours of operation are Monday through Friday from 8:00 a.m. to 4:30 p.m. excluding holidays.  The Bismarck HBPC office is located in the GSA Federal Building, 304 East Broadway, Bismarck, North Dakota.  The telephone number is 701-255-2048; the fax number is 701-255-2066.
