Circumstances:

Died while in MIA status. Declared MIA after an attack by Japanese planes on the USS Lindsey, near Mae Shima, southwest of Okinawa. Approximately one-third of the ship was blown away or torn and wrecked. One plane dove into her starboard side, and one minute later another crashed into the port side at the location of a gun mount magazine. The resultant explosion shattered the bow from keel to deck, blew the main deck upward at a 45-degree angle, started a gasoline fire and threw debris and shrapnel over a large portion of the ship.
