Adobe Acrobat ReaderIf you have trouble using any of these forms, you may be using an older version of Adobe Acrobat Reader. Download the current version by clicking the above icon.

Fill-in Forms Information

These forms are designed to allow you to complete them directly from the Web site by entering the data with your computer. If you prefer you may print out these forms and complete them by hand.

You will need the free Adobe Reader software in order to view, complete, and print these fill-in forms. Version 7.0 or later of Adobe Reader is recommended.

Beginning March 9, 2007, we are offering many of our fill-in forms with “Usage Rights” enabled. This feature is available beginning with the 2006 Individual Income Tax forms. Forms for prior years do not have this feature.

“Usage Rights” means that you will be able to save what you have filled in on your PC. Forms that have usage rights enabled will generate a dialog box when you open the form.
