Political Subdivision Payee Form

This form is used for entities:

• Cities
• Counties
• Government Agencies (federal or state)
• Schools
• Universities (state)

Complete one form for each Federal ID number your organization holds (SFN 54311). Include all addresses and/or relevant information concerning payments being issued to your organization in each form.

Foreign Government Payee

The W-8EXP is to be used by any of the following:

• Foreign governments
• Governments of U.S. possessions
