UCC Amendment (UCC-3/CNS-3)

Any amendment adding or changing the name of a debtor will be rejected if it doesn't contain the social security or tax identification number of the affected debtor.

Amendments, assignments, continuations, terminations, etc. of UCC filings must be filed in the same office the initial UCC-1 was filed.

The North Dakota Uniform Commercial Code Standard UCC-3/CNS-3 (SFN 14010) (40kb pdf) form or the National UCC Financing Statement Amendment form should be used.

The North Dakota form is designed to file a UCC-1 change and/or a CNS-1 change. The UCC-3/CNS-3 form can be filed with the UCC-5/CNS-5 overflow form.
