State Fleet vehicle accidents involving serious injury (death, potential for death, or potential for serious or permanent injury) or extensive property damage should be reported immediately to 9-1-1 and State Radio (1-800-472-2121) indicating that "this is a Risk Management accident."
All other incidents should be reported on-line to the Risk Management Division and State Fleet within 24 hours of the accident. You will find a paper copy of the Motor Vehicle Accident Report, SFN 51301 (324kb PDF), in State Fleet’s packet provided when the vehicle is checked out.
If the accident report is not filed on-line, the most current version of the paper forms are available online. The forms at this location are fillable on-line, but will need to be printed and filed with the Risk Management Division either by facsimile or mail.
The Risk Management Motor Vehicle Accident Review Board reviews every vehicle accident report involving vehicles being operated by state employees. The findings of the Board will be given to the employee and the Agency Head. If the Board determines that the accident was preventable recommended actions for the employee will be given to the Agency Head.
