This means that you can view them, type information directly into them, and then print them for signature. Please note that information typed into a fillable form will be lost when the document is closed. Therefore, you are advised to fully complete the form and print it before closing. Hoberg, Director If you fax the request and supporting documentation, you do not need to also mail the hard copy of your request.
