OAH Forms



The following three forms are the forms commonly used by the agencies requesting designation of an administrative law judge from the Office of Administrative Hearings to conduct hearings for the agency as the agency's hearing officer:

* Request for Administrative Law Judge - Complaint (fillable)
* Request for Administrative Law Judge - Appeal, Application, or Petition (fillable)
* Request for Administrative Law Judge - Rules Hearing (fillable)

The following form is used by agencies to request designation of an administrative law judge from the Office of Administrative Hearings to provide alternative dispute resolution:

* Request for Administrative Law Judge - Alternative Dispute Resolution (fillable)

About fillable forms: Most of the forms used by OAH are available on this web site in "fillable" format. This means that you can view them, type information directly into them, and then print them for signature. Please note that information typed into a fillable form will be lost when the document is closed. Therefore, you are advised to fully complete the form and print it before closing.

Please select the form you wish to use, fill it out to the best of your ability, and print it to either mail or fax, together with the supporting documentation, to:

If you fax the request and supporting documentation, you do not need to also mail the hard copy of your request. If you do not fax supporting documentation, you need to send it, along with a hard copy of the request, to OAH at the address above. If you have any questions about how to fill out the form, please call OAH at (701) 328-3260 before you fill out the request

Please remember: Only an agency head or a person specifically designated by the agency head to make hearing requests for the agency may make a request for the designation of an ALJ. Requests submitted by unauthorized persons will not be accepted for designation.
