Research policy form and rate filing records
Public access to policy, form and rate filing records
General information
A policy, form or rate filing is open for inspection weekdays between 8 a.m. to 5 p.m. provided the Department has completed its review of the filing.
Filings may be copied at a cost of 20 cents per page.
The Department does not retain filings for foreign insurance companies beyond 3 years. Domestic insurance company filings are kept perpetually.
Request to the Department
A person may request copies of the filings from the department. A request must include name and address (for billing purposes), phone number (in the event we need to contact you) and a detailed description of what you are looking for, including type (policy, form, rate or rule), company (if looking for a specific company), line of business ( private passenger auto, homeowner etc), and time frame (when filed or range of dates). Department staff will conduct the search, make copies and mail copies with invoice to the person requesting the records.
The Department will assist with simple searches. More involved searches should be done in person or referred to a private vendor.
