Company reporting forms
Here, you will find all forms needed for the company section of the North Dakota Insurance Department website. Please use the left-hand menu to find the forms that you are looking for. When filing, please use 8.5 x 11 (letter-sized) paper.
Licensed companies should read the general information under the filing requirements for licensed companies section.
PLEASE BE ADVISED THE ONLY RENEWAL REQUIREMENTS FORM THAT NEEDS TO BE FILED BY A COMPANY THAT IS "APPROVED OR REGISTERED" IN NORTH DAKOTA IS THE RENEWAL AFFIDAVIT FOR NON-ADMITTED COMPANIES ALONG WITH THE REQUIRED $10 FEE. NO OTHER FORMS ARE REQUIRED.
