NOTICE FROM LOUISIANA HEALTH PLAN FOR HURRICANE KATRINA VICTIMS

LHP advises that you can download an application for the Louisiana High Risk Health Insurance Pool or the HIPAA Pool at www.lahealthplan.org. Please check this site periodically for updates.

Please contact us at 1-800-736-0947 and press 0 for an Operator to assist you. If you know your party's extension, please dial the extension as soon as you hear the recorded message. Please leave a message if someone does not pick up. Messages are returned as soon as possible (or the next business day if it is after hours or on the weekend).

You will note that the HIPAA Pool and High Risk Pool applications can be downloaded and printed from this website.
