The identification supplement is used in conjunction with a self-issue single trip permit to facilitate the movement of a non-reducible oversize and overweight vehicle and load.   By obtaining identification supplements you will be able to use the self- issue single trip permits. (link to application for self-issue permits.)

Identification supplement for vehicle type:

1. Oversize and overweight- truck or truck/tractor and semitrailer hauling a nonreducible load.
2. Special mobile equipment - self propelled overdimensional/overweight vehicles (such as truck crane, pump unit...excludes earthmoving equipment.)
3. Work-over service rig

Identification supplements can be issued up to a maximum of:

Width - 14 feet 6 inches

Height - 15 feet 6 inches

Length - 120 feet

Overhang -   No limit

You will need a separate identification supplement for each vehicle and axle configuration that includes overweight authority. ie:

1. A 4 axle tractor pulling a tandem axle trailer or towed vehicle
2. A 4 axle tractor pulling a triple axle trailer or towed vehicle
3. A 4 axle tractor pulling a quad axle trailer or towed vehicle
4. Etc.

Gross vehicle weight - 150,000 pounds...truck or truck-tractor semitrailer

Axle weights:  See the axle weight chart on pages 3-4, in the Permit Policy for Oversize and Overweight Movements.    The combination weight of a drive axle with a helper dolly, or trailer axle with a stinger axle shall not exceed 68,000 pounds.

Once you have purchased and printed the identification supplement(s), you will receive an email with a link.  You MUST go to this link and print the required maps, policy and vertical clearances (if applicable.)  The required documents must be stapled to original identification supplement and placed in the truck.  Failure to attach the required documents automatically voids the identification supplement.

Single trip movement approval forms (self-issue permits) are used in conjunction with current identification supplements. A self-issue permit must be completed in its entirely prior to movement on the highway system. Self-issue single trip permits can be purchsed on-line using the NDHP E-permit System. You must have a current identification suppliement number. Self-issue single trip permits can also be obtained by completing the Application for Single Trip Movement Approval Forms  and fax or mail your request.

Oversize Identification supplement with vehicle type truck-tractor or truck must obtain Single Trip Movement Approval Form (3540).

Oversize identification supplements for Special mobile equipment must obtain Special Mobile Equip - Single Trip Movement Approval (16320) (excludes work-over service rig).

Oversize identification supplements for Work-over service rig  must obtain Special Mobile Equip - Single Trip Movement Approval (16320-WOR).
