A new "Surveys" button will be displayed on all service requests. ITD users will select this button to determine if a survey has been taken on the service request. One of the following values will be displayed:

• "N/A" : The requestor has not been given the option to take a survey on the service request.
• "No" : The requestor has been given the option to take a survey on the service request, but a survey has not been taken.
• "mm/dd/yyyy" : The requestor has taken a survey on the service request on the date listed.
