Requires licensure as a Licensed Social Worker (LSW) by the North Dakota Board of Social Work Examiners (NDCC 43-41); and one year of professional human services work experience as a Social Worker or Human Relations Counselor. OR two years of closely related experience as a Developmental Disabilities Case Manager, Mental Illness Case Manager, Vocational Rehabilitation Counselor, Activity Therapist, Addiction Counselor, Registered Nurse, Job Service Counselor, or a similar professional level position in the public or private sector. OR a Master’ Degree in Social Work and licensure as a Licensed Social Worker (LSW) by the ND Board of Social Work Examiners.

Summary of Work:
The Veterans Home provides care for a significant number of residents with mental health or addiction needs. The position requires flexibility in work hours to meet emergency needs of residents.
The Social Worker provides Social Work/Case Management Services to both units. Social Work duties include completing initial Social Services Assessments and ongoing MDS evaluation, Initial and quarterly Care Plan development, Admissions Procedures, assist residents with daily living concerns, provide ongoing support and counseling services to residents and their family members. Provide emergency counseling services as needed. Work with the other departments to implement and follow-up to provide the best care for the resident. Provide screening and prescreening services as directed. Coordinate, advise and report on resident groups, counsels, and/or organizations. Complete resident quarterly and annual reviews and document information as per policy and procedure. Provide residents with appropriate treatment and welfare needs. Complete forms, paper work, and charting according to the North Dakota Veterans Home, State and Federal regulations and Policies and Procedures. Complete quarterly Quality Assurance responsibilities as assigned. Provide in-service training as directed.
