SCOPE OF WORK:
The individual assigned this class is responsible for a
variety of duties related to processing of applications for admission
to both the basic and skilled care units of the North Dakota Veterans
Home. Additionally the individual performs a variety of administrative,
coordination, and counseling activities to facilitate resident adjustment
to the Home environment, on-going living support, and discharge planning.
DUTIES PERFORMED:
- Receive and document applications for admission and prepare
applications for review by the Admissions Board.
- Prepare and maintain status reports of application processing
and provide applicable notifications to applicants.
- Coordinate approved admissions with appropriate staff
of the facility; assure that admission documents are provided to assigned
unit.
- Maintain application log and waiting lists for Basic
and Skilled Care units.
- Provide information and assistance to prospective applicants
regarding admission requirements and procedures.
- Provide facility tours to prospective applicants and
other interested parties.
- Develop and maintain discharge plans; inform residents
of placement options and provide counseling regarding consequences of
choices; provide family and Veteran Service Officers with discharge
planning information and assistance in the decision-making process;
document discharge planning processes on resident’s Discharge
Summary Form.
- Provide counseling to residents and families relating
to financial, transportation, funeral/burial planning, application for
medical assistance, social security, and other Veteran’s Administration
benefits and assist in obtaining needed documents, legal assistance,
etc.
- Assist residents with daily living concerns including
resolution of complaints regarding living conditions, socialization
concerns, issues with staff and other residents, facility adjustment,
and orientation.
- Assist residents in understanding resident rights and
responsibilities and assist in obtaining and preparing documents relating
to living wills, power of attorney, guardianship matters, etc.
- Assist in development and maintenance of resident Social
Services assessments on all admissions; update assessments and social
history files, maintain social services care plans, and assist residents
in obtaining social services.
- Provide in-service training to county veteran services
officers and Veterans Home staff on Social Services and related services.
- Coordinate with safety officers, fire safety volunteers,
and facility support groups to ensure regulatory compliance.
- Attend and participate in workshops and in-service training.

Duties assigned any individual employee are at the discretion of the appointing
authority.
MINIMUM QUALIFICATIONS:
Requires four years of work experience in administrative
duties with extensive knowledge of specialized procedures for programs
or institutions, which includes responsibility for disseminating complex
information and instructions via written and verbal means. The hiring
authority may substitute college level coursework in a medical related
field for up to two years of the work experience requirement on a year-for-year
basis.
