Developmental Disability Service Provider: Report Documents

Developmental Disability Providers:

• These reports may be downloaded for your convenience. Please submit a printed and signed copy of the completed report to the DD Unit by the appropriate deadlines.
• Each year your organization will receive a cover letter indicating the budget report deadline. As indicated in NDAC 75-04-05-08.2.d., budget reports are due to DD within sixty days of the date of the cover letter.
• As indicated in NDAC 75-04-05-08(3)(a), cost reports are due 90 days from the last day of the provider's fiscal year.
