How to Apply for the Child Care Emergency Operating Grant
- STEP 1: Sign up in the Child Care Provider Self-Service Portal
- Online Provider Portal (Must be a ND Licensed or Self-Declared child care provider)

- Learn more about the Self-Service Portal used for both the Child Care Assistance Program and the CEOG (grant)
- Still having issues with the Self-Service Portal? Visit www.nd.gov/dhs/eligibility/need-help.html. (The baseline CEOG survey became available on the portal March 31.)
- For questions on using the Self-Service Portal, to create and register accounts, or for questions on Child Care subsidy, please email dhseap@nd.gov. Emails will be returned in the order received. You can also call 701-328-2332, however, preference is to contact via email.
- STEP 2: Complete the Baseline Survey
- Once your provider account (set up in Step 1) has been verified, you can log in to the Provider Portal to complete the "Baseline Survey."
- STEP 3: Complete the payment request form
- Every 2 weeks you will receive an email. This email contains a unique link for your program; it will be sent to the email address you entered in the Provider Portal.
- Providers MUST submit the payment request form no later than the deadline shown on the timetable below to be considered for payment. Late submissions will not be accepted.
- Grant requests will be reviewed and approved every 2 weeks according the time schedule below.
